Child's Birth Certificate or Confirmation of Birth from the Hospital
Child's Current Immunization Certificate (Form 3231)
Social Security Cards/Number for all household members
Parent or Legal guardian Photo I.D.
Child's Health Insurance Card (only if available)
Proof of Family Income for previous 12 months. Example proofs can be (All W-2s, Income Tax documentation such as 1040, 1040A, Pay Stubs (12 months' worth), Child Support (court order or previous 12 months payment statement), TANF award letter(s), SSI award letter(s)
IEP or IFSP documentation if applicable
Expectant Mother
Expectant Mother's Birth Certificate or Confirmation of Birth from the Hospital
Expectant Mother Proof of Pregnancy
Expectant Mother's Health Insurance Card
